Help for anyone with a Three60 Health login: providers, facility contacts, office staff and staffing partners. Three60 Health is run by Zenith Elevation LLC.
Can't sign in?
- On the sign-in screen, press Forgot password? to set a new password, or Email me a sign-in link to get a one-time link.
- Accounts are made by your organization's scheduling office. If you don't have one yet, ask them to invite you.
Your schedule, hours or documents look wrong
Your organization's office keeps those records and can correct them. Send them a message in the app, or contact them the way you usually do.
Notifications
This version of the app sends notifications by email, to the address on your account. You can see that address under Account.
Contact us
Email chris@zenithelevation.com. Tell us what you were doing and what you saw. Please don't include any patient information.
Delete your account
You can delete your account yourself in the app, under Account. How deleting works.
