Terms of Service

What this is. Three60 Health is scheduling, credentialing record-keeping, timesheet, and billing-handoff software for medical provider groups and staffing operations, operated by Zenith Elevation LLC of Broken Arrow, Oklahoma ("Zenith," "we"). An organization subscribes and its administrators create accounts for staff, providers, facility contacts, and staffing partners. By creating an organization or signing in to an account, you agree to these terms. If your organization has signed a separate written agreement with Zenith, that agreement governs your organization's subscription, and these terms apply to your personal use of your account only where they do not conflict with it.
Organizations and administrators. The organization that creates a tenant is responsible for everyone it invites: for assigning roles that match each person's job, for removing accounts when a working relationship ends, and for what its users do in the system. Administrators can view, edit, and delete records in their organization's tenant, including records about you, as part of running staffing operations.
Your account. Keep your login private and tell your administrator or us right away if you think someone else has used it. Two-factor authentication is available to every user; staff who can see provider documents should turn it on. You must be at least 18 to hold an account.
Acceptable use. Use the system only for legitimate scheduling, credentialing record-keeping, timesheets, billing, and staffing operations connected to your role. Do not share logins, try to reach data outside your role or organization, probe or disrupt the service, run automated access against it other than the exports and calendar feeds we provide, or upload content unrelated to staffing operations, unlawful content, or malicious code.
No patient information. The system is built for staffing operations and is not designed to hold patient information. Do not enter patient names, identifiers, diagnoses, or clinical details anywhere in the system, including notes, messages, uploads, spreadsheet imports, and document imports. Zenith is not a business associate of any organization using the service, and no business associate agreement is created by these terms.
Credentialing records. Credentialing features track, organize, and date-check the records you or your organization add. We do not verify any license, certification, or registration against a primary source, do not confirm anyone's standing with a licensing board or registry, and do not decide whether anyone is qualified to work. Readiness labels such as "Ready," "Needs attention," and "Blocked" reflect the records on file and their dates, nothing more. Primary-source verification stays with your organization's credentialing office.
AI-assisted features. Smart Document Import reads uploaded credential documents into suggested field values, spreadsheet import suggests column mappings, and the matching advisor suggests candidates for a shift. Every suggestion is reviewed and confirmed by a person before anything is saved. We do not guarantee the accuracy of any suggestion, and you are responsible for what you confirm. Manual entry is always available if you prefer not to use these features. See the Privacy Policy for how documents are processed.
Scheduling, billing, and messages are your decisions. Conflict detection, matching, and readiness indicators are aids; your organization's staff make every scheduling and assignment decision. Billing batches and exports are built from the timesheets and rates your organization enters, and your organization is responsible for reviewing them before invoicing anyone. Emails, messages, reminders, and documents sent through the system are sent by your organization, which is responsible for their content and for having permission to contact the recipients. Zenith is not a staffing agency, employer, or placement service and has no employment relationship with any provider.
E-signed agreements. Organizations can send documents through the system for electronic completion and signature. The system records consent, the signer's typed name, time, IP address, device, a fingerprint of the document text, and a signed PDF. The agreement is between the organization and the signer; Zenith is not a party to it and makes no promise about the enforceability of any particular document or template.
Subscriptions and fees. Standard and Pro plans are priced per provider on your roster with a monthly minimum, are prepaid each month, and continue month to month until cancelled. You can cancel from your account at any time; the cancellation takes effect at the end of the paid month and we do not refund partial months. Enterprise plans are billed monthly in arrears on actual roster under a signed agreement with a 12-month minimum term. Prices are in US dollars and exclude taxes. ACH or bank transfer is our preferred payment method and carries no processing cost. If you choose to pay by card, we may add the actual cost of card processing as a separate, clearly labelled line where the law allows it, never above our cost or any legal cap, and never on a debit or prepaid card. We may change plan prices with at least 30 days' notice before the change takes effect; the change applies to your next renewal. Accounts more than 30 days past due may be suspended after notice.
Your data. Your organization owns the data and documents it and its users put into the system. You give us permission to host, process, back up, and display that data to authorized users in order to run the service, keep it secure, and support you. Administrators can export their organization's data at any time. After a subscription ends we keep the tenant available read-only for 60 days for export, then delete the data from the live service within 30 days; backup copies age out within a further 30 days. We may remove content that violates these terms.
Our software. Zenith owns the platform, its design, and everything in it other than your data. These terms give you the right to use it, not to copy, modify, reverse engineer, resell, or build a competing product from it. Suggestions you send us can be used to improve the service without any obligation to you.
Text messaging. Text messaging is optional. You agree to receive texts only by opting in, as described in the Privacy Policy. Reply STOP to any message to end texts and HELP for help. Message frequency varies with your schedule and your organization's settings, and message and data rates may apply. Carriers are not liable for delayed or undelivered messages. Your organization controls whether text messaging is enabled and which notices it sends.
Mobile apps. The Android and iOS apps are companions to the web application and are subject to Google Play's and Apple's App Store terms as well as these. Camera capture and push notifications are optional and controlled by your device settings.
Availability and disclaimer. We work to keep the service available, accurate, and secure, and we publish backup status in the app. The service is still provided "as is" and "as available," without warranties of any kind, express or implied, including merchantability, fitness for a particular purpose, and non-infringement. We do not promise uninterrupted or error-free operation, that any credential record is valid or current, that any shift will be filled, or that using the service will produce any particular regulatory or business result.
Limitation of liability. To the fullest extent the law allows, Zenith is not liable for indirect, incidental, special, consequential, or punitive damages, or for lost profits, revenue, or data, arising from the service or these terms. Our total liability to you or your organization for anything arising from the service is limited to the fees your organization paid us in the 12 months before the event giving rise to the claim, or $100 if your organization paid nothing. Some jurisdictions do not allow certain limitations, so parts of this section may not apply to you.
Responsibility for your use. Your organization is responsible for claims that arise from its data (including any patient information it should not have uploaded), its use of the service in violation of these terms or the law, its staffing, credentialing, scheduling, and billing decisions, and the communications and documents it sends through the service, and agrees to defend Zenith against such claims.
Suspension and termination. Administrators may suspend or remove accounts at any time, including at the end of a working relationship. We may suspend or end access for violations of these terms, for non-payment, or to address a security risk, with notice where practical. Provisions that by their nature should survive (your data, our software, disclaimers, limitation of liability, responsibility for your use, and governing law) survive termination.
Governing law. These terms are governed by the laws of the State of Oklahoma. Disputes will be brought in the state or federal courts sitting in Tulsa County, Oklahoma, and both sides consent to that venue. If any part of these terms is unenforceable, the rest still applies.
Changes. We may update these terms. We will post the new effective date here and, for material changes, show a notice in the app. Continued use after the effective date is acceptance.
